Definition: Autism is a neurobiological developmental disorder that manifests itself through differences in social interaction, communication, stereotypical behavior and special interests. The autism spectrum encompasses many different manifestations and can affect people at all stages of life. The causes are complex and predominantly genetic. The aim of diagnosis, counseling and support is to improve quality of life, support individual strengths and enable social participation. A complete cure is not possible, but targeted support from specialized professionals is.

Concept and classification

Autism spectrum disorders (ASD) are profound developmental disorders that are defined as a separate group of disorders in the international classification systems (ICD-10: F84.0-F84.9; DSM-5: „Autism Spectrum Disorder“). They are characterized by impairments in social interaction, communication and restricted, stereotypical behavior patterns and interests. The term „spectrum“ emphasizes that autism occurs in different degrees and forms.

Epidemiological data and relevance

Autism spectrum disorders occur in around 1-2 % of the population, although the incidence has risen in recent decades due to improved diagnosis and increased awareness. Boys are more commonly affected than girls, with girls often showing more subtle symptoms that are not recognized until later. ASD is a lifelong developmental disorder that requires individual support at different stages of life.

Genetic and neurobiological factors

The causes of autism are multifactorial. Genetic factors play a central role: numerous risk genes have been identified which, in combination, increase susceptibility. Twin and family studies show a high degree of heritability. Neurobiologically, abnormalities in brain development, the networking of brain regions and the processing of social information are being discussed. Environmental factors during pregnancy (e.g. infections, certain medications) can influence the risk, but are not the sole cause.

Psychological mechanisms and information processing

People on the autism spectrum often process information in a detail-oriented and less intuitive way in social contexts. Difficulties arise in particular with the „theory of mind“ - i.e. intuitively grasping the thoughts, intentions or emotions of other people. Many of those affected show a pronounced perceptual sensitivity to stimuli such as sounds, light or touch, which can lead to excessive demands. At the same time, there are often pronounced interests in specific topics.

Clinical picture: Social interaction and communication

It is typically difficult to intuitively understand and use non-verbal signals such as eye contact, facial expressions or gestures. Conversations can seem one-sided or formal. Some of those affected speak little or not at all, others have age-appropriate, sometimes even above-average language, which is, however, limited in its social application. Friendships are often experienced differently: the need for social contact may be present, but the intuitive skills to form it are lacking.

Stereotypical behavior patterns and special interests

Repetitive behaviors, routines or rituals are a central characteristic. Many children and adults with ASD develop intense special interests that they pursue with great persistence. Changes in the daily routine or in the environment can cause severe restlessness and stress. Motoric peculiarities such as „flapping“ (hand flapping) or rhythmic rocking are often used for self-regulation.

Cognitive characteristics

Cognitively, the spectrum is broad: many of those affected have average intelligence, some have reduced intelligence, others show above-average abilities in certain areas (e.g. pattern recognition, mathematics, detailed memory). These isolated talents („savants“) are rare, but are often overemphasized in the media. Their strengths often lie in accuracy, logical thinking and perseverance in routine tasks.

Psychosomatic accompanying phenomena

Many people with autism have additional physical or psychological stresses. Sleep problems, gastrointestinal complaints and increased stress reactions are common. Due to the sensitivity to stimuli and difficulties in expressing needs, physical complaints can go unrecognized for a long time. Comorbid anxiety disorders, ADHD or obsessive-compulsive disorders often occur.

Differentiation from other developmental disorders

The diagnosis requires a careful differential diagnosis to language development disorders, social anxiety disorders or ADHD. The qualitative impairment of social communication and interaction since early childhood is essential. In contrast to attachment disorders, autism is not based on relationship experiences, but on neurobiological characteristics.

Children and young people

In children, the first abnormalities are often noticed between the second and third year of life: lack of eye contact, lack of pointing, hardly any shared joy. In kindergarten and school age, social difficulties and stereotypical interests become more apparent. Adolescents with ASD often develop compensatory strategies, but these reach their limits when faced with new social demands (puberty, first relationships).

Diagnosis in practice

The diagnosis requires a comprehensive developmental history, observation of social interaction and standardized diagnostic procedures (e.g. ADOS, ADI-R). Parents, teachers and other caregivers are involved. Early diagnosis facilitates access to remedial measures and individual support.

Progression and prognosis

Autism lasts a lifetime, but many of those affected develop coping strategies over time. Early support, an adapted environment and acceptance promote social participation. The progression depends heavily on cognitive abilities, language development and available resources. Some people lead largely independent lives, others require lifelong support.

Treatment: support and therapy

There is no causal cure, but there are many effective support approaches. Early intervention, behavioral therapy, social communication training and occupational therapy improve practical everyday skills. AAC or structured learning environments help with language or cognitive impairments. Parent training is important to understand and support behavioral problems.

Medication support

Medication can be used for accompanying problems such as severe hyperactivity, anxiety or self-injurious behavior. They do not treat the autism itself, but symptoms that severely impair quality of life. Medication should be weighed up individually, monitored by a doctor and dosed as low as possible.

Self-help, prevention and the role of the environment

An understanding, structured and predictable environment is crucial for well-being. Parents, teachers and caregivers benefit from education and advice. Self-help groups and autism associations offer exchange and practical tips. Socially, inclusion is key: acceptance and appropriate support services enable people with autism to lead a self-determined life.

Frequently asked questions & answers

How do I recognize autism in children?

Autism often manifests itself from the age of two - e.g. lack of eye contact, no pointing or little shared joy. Stereotypical behavior and social abnormalities often occur at kindergarten age. A precise assessment is carried out using standardized diagnostic procedures.
No, autism is not a disease, but a lifelong developmental peculiarity. There is no „cure“ in the traditional sense - but there is targeted support to better overcome challenges and promote individual strengths.
Diagnostics are carried out by specially trained specialists, usually as part of an interdisciplinary assessment. Structured procedures such as ADOS or ADI-R are used. Parents, teachers and other caregivers are involved.
